Consumer Health Data Privacy Policy
Our notice about care-related information and your Washington consumer-health-data rights.
Effective September 20, 2026
Who this notice covers
Elavise LLC, doing business as Care Homes Washington, operates CareHomesWA.com. This notice describes personal information that reveals or can reasonably be linked to a person’s health status or efforts to obtain care, where protected by Washington’s My Health My Data Act.
Categories collected and purposes
- Care needs: memory care and dementia, personal care and daily living, behavioral and mental health, or developmental disabilities, as selected in the personalized-help form. We use the selected category to understand and respond to the request.
- Care-seeking preferences: preferred location, budget range, payment preference such as Medicaid or private pay, and move-in timeline. These help us understand the assistance requested.
- Associated contact and request information: name, email, phone, consent and submission timestamps, and handling status. These connect the request to the person we should contact and support follow-up.
- Search, location, and technical information: entered locations, optional device coordinates, facility pages requested, IP addresses, and browser information may indicate care-seeking activity when reasonably linked to an individual. These support location lookup, distance calculations, maps, website delivery, troubleshooting, and security.
- Care-related correspondence and earlier submissions: information you include in messages and any retained care-related content from the retired review feature. This is handled as part of the correspondence or retained submission, not a new public review feature.
Sources
Information comes from you, an authorized family member or representative submitting information for you, the browser or device accessing the site, and mapping or address-lookup services returning location results.
Disclosures and recipients
Internal family inquiries: CareHomesWA uses family inquiries to respond to the family. We do not publish them or distribute names, emails, phone numbers, or care needs to facility owners, sponsors, referral businesses, or affiliates for their own use. Families decide whether to contact a facility themselves.
Service providers: hosting, database, storage, and communication providers may process care requests, associated contact information, messages, and technical data needed to operate the service. Authorized CareHomesWA administrators handle inquiries.
Mapping and address lookup: Google Maps receives browser requests and information used to display the selected area, including coordinates when device-location search is used. OpenStreetMap/Nominatim receives typed location queries and connection information. These can include location and care-seeking context. The care-help form submits its full set of answers and contact details to CareHomesWA; it does not send the completed form to these mapping services.
Map and location providers operate under the Google Privacy Policy and OpenStreetMap Foundation Privacy Policy. Information may also be disclosed where required by law, subject to applicable protections for consumer health data.
Your rights and requests
Email hello@carehomeswa.com with “Health data privacy request.” You may request access, confirmation of collection/sharing/sale, recipient identities and contact details, consent withdrawal, or deletion. No new account is required; reasonable identity or representative-authority verification may be needed. Avoid including detailed medical records.
Under Washington law, requests must be addressed within 45 days of receipt; a permitted extension adds up to 45 days, with notice during the first period. Information is generally free twice yearly, subject to statutory exceptions. Deletion includes notifying relevant recipients; permitted backup delays cannot exceed six months after authentication. Withdrawing permission may prevent continuation of personalized assistance. Exercising these rights must not lead to unlawful discrimination.
To appeal a denial, email the same address with “Health data privacy appeal.” A written appeal decision is due within 45 days. If denied, you may complain to the Washington Attorney General. See RCW 19.373.040.
